2 Simple Steps to Prepare for Your Appointment
1. Watch this 30-minute video
Are you enrolling for the first time, or re-enrolling after two years? Then you must watch this 30-minute orientation video before your appointment. If you don’t watch the video before your appointment, we may need to reschedule you.
You can also watch the video in Spanish, or with subtitles in Arabic or Vietnamese.
2. Gather Your Documents
Please have the following documents ready during your appointment. If you don’t have any one of these documents, please keep your appointment. Your counselor will work with you on a solution.
For example:
- Paystub
- LES for military families
- Medi-Cal number
For example:
- A bill that has your address in California on it
- A piece of mail that has your address in California on it
- ID or drivers license with your address in California on it
- Medi-Cal card with your address in California on it
For example:
- ID, drivers license or passport
- Birth certificate
- Insurance card
For example:
- Ultrasound with your name on it
- Medical records
- Letter from your healthcare provider
If you don’t have these results, please still keep your appointment.
